Wisdom tooth removal – when lack of space becomes a problem

Wisdom teeth are often the cause of a wide variety of problems. Originally designed for a larger jaw, they often no longer fit into our modern dentition. If they remain completely or partially stuck in the jaw or erupt crookedly, they can affect the entire dentition.

Surgical removal may be advisable in the following cases:

  • if they are anchored in the jaw and do not erupt

  • in cases of severe caries that can no longer be treated

  • for gum pockets that are difficult to clean

  • if they jeopardise orthodontic results

  • in cases of malocclusion that make oral hygiene difficult

  • if they interfere with dental prostheses

At our practice in Heidelberg, we remove wisdom teeth using modern, gentle techniques – with sedation available on request.

Tooth preservation through surgical procedures

If a tooth causes discomfort despite root canal treatment or if the surrounding bone is inflamed, surgical procedures are used to preserve the tooth:

  • Apicoectomy: Removal of the infected root tip
  • Root amputation: Removal of a single tooth root (especially in maxillary molars)
  • Hemisection: Sectioning and removal of a root portion in lower molars
These procedures usually follow a root canal treatment and aim to preserve the healthy remaining tooth structure while stopping the inflammation.

Frequently asked questions about surgical dentistry in Heidelberg

No, only if complications arise or are expected. We assess each case individually.

The tooth is surgically removed under local anaesthetic or, if desired, with sedation. The procedure usually takes 20–40 minutes.

The wound healing process takes approximately 1–2 weeks. After that, we regularly check the healing progress using X-rays.

When part of the tooth is no longer worth preserving, but the other root is healthy – especially in the case of lower molars.

As with any surgical procedure, there are minor risks of infection, post-operative bleeding or swelling – however, these are rare and easily manageable.
